Limitless Me Therapy (“I”, “me”, “my”) is committed to protecting your privacy and handling your personal information in accordance with the Privacy Act 1988 (Cth) and the Australian Privacy Principles (APPs).
What I collect
I collect personal information you provide through my website enquiry and referral forms, by email, phone, or during our work together. This may include your name and contact details, and — because I provide health services — sensitive information such as health, disability and NDIS-related information. Where you refer another person, I rely on you having their consent to share their information.
How I use it
I use your information to respond to enquiries and referrals, provide occupational therapy services, communicate with you, prepare reports and documentation to support your NDIS plan, and meet my professional and legal obligations.
Disclosure
I only disclose your information where necessary and with your consent — for example, to your plan manager, support coordinator, GP or other providers involved in your care, or where required by law. I do not sell your information or disclose it overseas.
Storage and security
I store your information securely and take reasonable steps to protect it from misuse, loss and unauthorised access. Records are retained as required by professional and legal obligations.
Access and correction
You may request access to the personal information I hold about you, and ask me to correct it if it’s inaccurate. Contact me using the details below.
